Working closely with our reablement providers and the voluntary sector your supervisees will support and guide people to regain independence in their own home using a strength-based approach to goal planning during short term intermediate care provision. This intervention will be provided following discharge from hospital or to help prevent admission to hospital.
You will be responsible for overseeing the practice of a group of team members, both registered professionals and non registered professionals, supporting their development and skills to ensure delivery of timely reablement interventions reducing reliance on statutory care. You will be working closely with colleagues in Health and the voluntary sector to ensure whole system flow is achieved and sustained. This will require a flexible and proactive approach to collaborative working as demand fluctuates. A background in community working with strong moving and handling skills will be essential.

This role will require you to be on an on call rota a maximum of one in 5 weekends and 2 bank holidays in a 12-month period. Your annual salary will be enhanced to reflect this.
