To act as a named EHCP Coordinator from referral , assessment, support planning and review stages in line with the SEND Code of Practice (January 2015), for individual children/ young people (up to the age of 25) subject to an Education Health and Care assessment/plan or statement of Special Educational Needs .
To support a caseload of children and young people with special educational needs within the team.
To supervise EHCP Coordinators.
To casework children and young people with special educational needs who are deemed to have complex needs/challenging circumstances.
To communicate the Council’s vision “Lives not Services”, values and strategic objectives in all aspects of work.
To support EHCP Coordinators to undertake Education Health and Care assessments, planning and placement processes within statutory timescales, reaching appropriate placement decisions for children and young people aged 0-25.
To line manage, support and appraise EHC Plan Coordinators.
To coordinate identified transitional arrangements, for example Junior to Secondary.
To coordinate specific strands of work activity such as Local Offer, Children in Care, Preparing for Adulthood, Personal Budgets, Children in Independent Provision, Children Missing School, Special Schools across the SEN Team, as directed by the SEN Manager. In addition, to monitor and evaluate these.
To act as the EHC Plan Coordinator for children and young people with complex needs, challenging circumstances, or those in independent provision. To present such children and young people’s needs to panels with a view to securing multi-agency resources. To consult, negotiate and arrange the placements for this group of children. To support the EHCP Coordinators in the dissemination of SEN Panel decisions to parents, referrers, educational settings appropriately.
Qualifications: Relevant professional qualification (Teaching, Psychology, Child Care, Social Work).
Experience: 2+ years working within a similar role.
Compliance: Current DBS check.
Travel: Clean UK driving license with the ability to commute to and from work independently.
